Case Manager

Case Manager, I

Location: Tulsa, OK
Job Type: Part-time
Reports to: Program Director


Position Summary:

The Case Manager plays a critical role in supporting residents through recovery, stability, and successful reintegration while participating in Thrive Transitional Housing. This position is responsible for coordinating services, assisting residents with individualized goal planning, connecting residents with community resources, monitoring program compliance, and maintaining accurate documentation. The Case Manager works closely with residents, treatment providers, probation officers, case workers, and community agencies to help residents achieve long-term success and independence.

The ideal candidate is organized, compassionate, trauma-informed, and capable of maintaining professional boundaries while providing structured support and accountability.


Key Responsibilities:


Resident Case Management & Support:

  • Conduct resident assessments and assist with individualized service planning and goal setting.
  • Provide ongoing support, advocacy, and accountability to residents participating in the program.
  • Assist residents with accessing community resources including employment, identification documents, healthcare, transportation, food assistance, outpatient treatment, and recovery services.
  • Conduct regular resident check-ins to monitor progress, barriers, and overall well-being.
  • Encourage participation in recovery meetings, outpatient services, employment, and program expectations.
  • Support residents in developing healthy coping skills, life skills, and independent living skills.
  • Maintain a trauma-informed and person-centered approach in all interactions.


Program Compliance & Coordination:

  • Monitor resident compliance with program rules, curfews, recovery plans, and house expectations.
  • Coordinate with probation officers, treatment providers, referral agencies, and case managers as needed.
  • Assist with intake, orientation, and discharge planning for residents entering or exiting the program.
  • Address resident concerns professionally and report serious issues or policy violations to management.
  • Participate in case staffing meetings and collaborate with the Thrive team regarding resident progress and needs.


Documentation & Administrative Duties:

  • Maintain accurate and timely case notes, service documentation, referrals, and incident reports.
  • Ensure documentation meets billing, compliance, and program standards.
  • Track resident progress toward goals and required program participation.
  • Maintain confidentiality and professionalism in all resident records and communications.
  • Utilize technology systems for documentation, scheduling, and communication purposes.


Qualifications & Skills:

  • Experience in case management, social services, behavioral health, recovery support, or related fields preferred.
  • Strong organizational, communication, and interpersonal skills.
  • Ability to work effectively with individuals experiencing substance use, mental health challenges, trauma, and justice involvement.
  • Knowledge of community resources and recovery support services.
  • Understanding of trauma-informed care, recovery principles, and structured living environments.
  • Ability to maintain professional boundaries while remaining supportive and compassionate.
  • Reliable transportation required for travel between housing locations and appointments if needed.
  • Organized, dependable, and capable of maintaining detailed documentation.
  • Basic technology proficiency for record-keeping and communication.
  • Must pass a background check and be comfortable working within structured recovery housing environments.


Preferred Qualifications:

  • PRSS certification preferred but not required.
  • Lived experience in recovery preferred but not required.
  • Experience working with justice-involved individuals, sober living, reentry, or behavioral health programs preferred.
  • CPR/First Aid certification (or willingness to obtain).


Work Environment & Schedule:

  • Flexible schedule – may require evenings, weekends, and occasional on-call availability.
  • Combination of office work, resident interaction, and community-based support services.
  • Regular interaction with residents at housing locations and external agencies.
